Requirements

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

Responsibilities

  • Provide immediate, trauma-informed crisis intervention and emotional support to patients and families in the Emergency Department following incidents of violence.
  • Coordinate comprehensive case management, including safety planning, assessing social determinants of health (SDoH), and connecting patients and families with essential hospital and community resources.
  • Mediate potential conflicts and negotiate peaceful resolutions to prevent escalating and retaliatory violence.
  • Manage an active caseload of 10-12 clients, providing continuous support to prevent future victimization and reduce the risk of violence in the greater community.
  • Maintain accurate documentation of all patient interventions and outcomes to support program reporting and drive continuous improvement in intervention strategies.
