This position ensures the implementation of the WV Birth to Three (WVBTT) Individualized Family Service Plan process. Key functions include contacting assigned referrals within established timelines, informing families of WVBTT Procedural Safeguards, and establishing and maintaining working relationships with Service Coordinators, practitioners, and community partners within the Regional Administrative Unit’s geographical service area. The role also involves coordinating transition planning activities/conferences with other early childhood programs and facilitating initial intake, eligibility meetings, and Individualized Family Service Plan (IFSP) meetings. The coordinator will also inform families and Service Coordinators of the travel reimbursement process, compile and submit reports, assist in recruiting practitioners, participate in ongoing monitoring and continuous improvement activities, and actively engage in Child Find and public awareness activities. Responding to inquiries and participating in required committees, conferences, meetings, and trainings are also part of the role.
