About The Position

The Intensive Family Coordinator provides comprehensive case management and supportive services to children, youth, and families requiring higher‑level, wraparound‑style intervention. In this role, the Coordinator works closely with families to assess needs, develop individualized service plans, and connect them with community resources that promote safety, stability, and long‑term well‑being. This position requires strong advocacy skills, consistent communication with service providers, and the ability to support families through complex behavioral, emotional, or environmental challenges. The Intensive Family Coordinator plays a key role in ensuring families receive cohesive, strengths‑based support aimed at maintaining children safely in their homes and preventing unnecessary placement. The Intensive Family Coordinator will be responsible for completing family assessments and creating individualized service plans. The Intensive Family Coordinator will report to the Director of the Program. Successful applicants will possess outstanding people skills, creativity, and energy as well as expertise of Tompkins County. Intensive Family Coordinator provides a professional level of case management services as well as direct advocacy services to a caseload of families. Intensive Family Coordinator will be responsible for completing family assessments, recruiting foster parents, completing home studies for the referring authority, locating family resources, and creating individualized service plans. Case management services including intakes, safety planning, team meetings, ISP development 24/7 support, case contacts, and case record documentation in keeping with state regulations. Travel will be necessary Flexible Schedule with willingness to work evenings and weekends and provide 24/7 crisis support to the caseload as needed.

Requirements

  • High School Diploma or Equivalent is required.
  • At least 1-2 years of experience working with children, youth, and families in social services, behavioral health, child welfare, or community-based support programs.
  • Experience providing case management, family support, crisis intervention, or coordination of services is strongly preferred.
  • Strong organizational and time management skills
  • Proficient in Microsoft Office 365; knowledge of electronic health record (EHR) systems is a plus.
  • Position requires reliable transportation, valid driver’s license, and current auto insurance coverage.

Nice To Haves

  • Bachelor's degree in social work, psychology, human services, or related field is preferred.
  • Bilingual (Spanish) speaking is a plus
