About The Position

The Intensive Case Manager (ICM) provides appropriate intensive case management services to refugees or other eligible immigrant populations who have resided in the US for less than 5 years. The ICM program fosters the successful and integration of especially vulnerable refugees, asylees, Special Immigrant Visa (SIV) holders, Afghan/Ukrainian/Cuban/Haitian Humanitarian Parolees. The ICM program aims to effectively support vulnerable individuals that have difficulty navigating systems and attaining self-sufficiency in their new or existing community. The ICM case manager will provide intensive support to large families, women-at-risk, elderly refugees, refugees that identify as LGBT, refugees with significant physical or mental disabilities, and other vulnerable populations as needed. Position Details: Full Time position; Hourly Department – Refugee and Immigrant Services Some on-site, remote and field days required Requires driving and transporting clients to appointments as needed Reports to the Atlanta Client Services Manager Located in Atlanta, GA Mostly Virtual/Remote with some on-site required, Travel 50% locally Monday through Friday; 8:30AM – 5:00PM Hourly compensation $19.23-$21.63

Requirements

  • Bachelor’s degree in social work, counseling, public health, psychology, or a related field required; Master’s degree preferred.
  • Minimum of two years of direct service experience in a culturally diverse environment, or an equivalent combination of education and experience.
  • Demonstrated experience in case management, health advocacy, community outreach, and partnership development preferred.
  • Knowledge of immigration and/or refugee issues and familiarity with medical, insurance, maternal and child health, or reproductive health systems preferred.
  • Strong organizational skills with the ability to manage multiple client-related activities and competing priorities.
  • Excellent written and verbal communication skills in English; bilingual or multilingual proficiency in a relevant language preferred.
  • Demonstrated cultural competence and sensitivity to diverse cultural and socioeconomic backgrounds.
  • Ability to maintain professional boundaries, set appropriate limits, and balance client advocacy with organizational and community expectations.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and strong general computer and documentation skills.
  • Valid Georgia driver’s license; reliable multi-passenger vehicle, personal auto insurance, and good driving record required.

Responsibilities

  • Provide intensive, client-centered case management beyond the initial resettlement period, addressing complex medical, educational, housing, and special needs while developing individualized, holistic service plans.
  • Conduct RHS-15 mental health screenings and ensure timely, appropriate referrals to behavioral health and other specialized service providers.
  • Build and maintain strategic partnerships with mainstream and refugee-serving agencies; develop and manage a comprehensive community resource database to enhance service delivery.
  • Deliver health orientation and public health education, including Medicaid navigation, provider selection, and language-access resources; coordinate appointments and support clients in resolving urgent or emergency needs.
  • Develop and facilitate community support groups to promote social connection, healthy coping strategies, and integration; collaborate with Community Engagement staff to connect clients with volunteer mentoring programs.
  • Maintain clear communication with leadership regarding complex cases; uphold professional boundaries, ethical standards, and mutually respectful partnerships with clients, colleagues, volunteers, and community stakeholders.
  • Ensure compliance with organizational policies and all applicable federal, state, and local regulations; actively participate in training, quality improvement initiatives (PQI), public relations, and agency-wide engagement activities while striving for excellence and innovation.
