The Integrated Behavioral Health Care Manager is a core member of the Integrated Care team, working closely with the patient's medical and behavioral health providers, other care managers and the larger primary care team and staff. The Integrated Behavioral Health Care Manager is responsible for supporting and coordinating the mental and physical health care of patients with the patient's medical and behavioral health providers. In coordination with medical and behavioral health providers, this position, monitors and tracks treatment response for changes in clinical symptoms and treatment side effects or complications. Supports and encourages individuals to increase knowledge about their mental and physical health conditions, engage in their care, and improve adherence to treatments. Provides patient education about common mental health and substance abuse disorders and the available treatment options. Facilitates referrals for clinically indicated behavioral health services and substance abuse treatment outside of the organization and coordinates with other care managers and primary care staff to facilitate referrals for other needs (e.g., social services such as housing assistance or vocational rehabilitation). Supports patient medication management including monitoring adherence and ability to access obtained medications. Assists with managing patient appointments and phone calls for Integrated Behavioral Health Care Team, in coordination with medical providers and support staff. Provides support to patients where psychosocial barriers interfere with their ability to access or maintain recommended treatments for behavioral health concerns.
