We are seeking a full-time Behavioral Health Clinician to be part of our fast-paced Patient Centered Primary Care team and their patient panels at La Clinica's Southern Oregon University Health Center. We are dedicated to the primary care behavioral health (PCBH) integration model and support early learner Integrated Behavioral Health Clinicians (IBHC) by offering: 4-hrs/month group and individual clinical supervision which meets licensing Board requirements Opportunities for experiential didactics/case conferences with a PCBH focus Monthly offerings to shadow both internal and external continuum of care service providers  The Integrated Behavioral Health Clinician (IBHC) is part of the primary care team and works closely with the primary care provider, nurse, medical assistant, and other team members targeting behavioral health needs to include the mental health and substance use challenges of our patient population. The IBHC provides evidence-based time-limited assessment, consultation, and therapy services to promote behavioral health across the lifespan through easy-access, excellent interdisciplinary teams, and integrated care to children, teens, adults, and families, both individually and in group settings making referrals when necessary. Our IBHCs work in collaboration with the primary care team, as well as community agencies to foster patient wellness and satisfaction in a warm, therapeutic, and respectful manner.   Our primary care teams are diverse, and some may hold specialty services such as maternal fetal medicine, dental, and/or additional specialty care populations.  Training and experience in working in medical settings as part of a multidisciplinary team as well as experience providing evidence-based behavioral health interventions are desired. Requirements

  • Master's degree in Social Work, Psychology, Marital and Family Therapy, or Clinical Mental Health Counseling
  • Skilled at using electronic health records as applicable to area of work (EPIC) 
  • This position requires you to be a Licensed Clinical Social Worker, Licensed Professional Counselor (LPC) or Licensed Marriage and Family Therapist (LMFT) 
  • A Registered Clinical Social Work Associate (CSWA), a Professional Counselor Associate (PCA) or a Marriage & Family Therapy Associate (MFTA) with the State of Oregon (we provide Clinical Supervision).  We will support you in becoming a registered associate and offer you clinical supervision toward licensure.

Responsibilities

  • As a collaborator with La Clinica medical and dental providers, ensures delivery of comprehensive clinical services to primary care patients that target interventions to improve patient functioning, behavior and clinical outcomes 
  • Responds to the referring Provider’s referral concern or question and provides focused feedback in both written and verbal form
  • Identifies and evaluates health risk behavior(s), assesses and diagnoses mental health and substance use conditions, develops care plans, and provides brief, targeted interventions to support optimum mental, behavioral, emotional and physical health  
  • Participates in offering stepped behavioral health care to patients, collaborating with the Psychiatric Nurse Practitioner in La Clinica for psychiatric medication management needs and referring to specialty mental health and substance use care when warranted  
  • Supports team as needed around assessing and intervening for patients at risk of harming themselves or others, domestic violence. Identify a safety plan and refer to appropriate services 
  • Participates in patient care staffing(s) with the clinical care team
  • Provides clinical consultation and psycho-educational programs on behavioral health related topics to staff, students, consumers, and other agencies as requested
  • Participates in clinic and performance improvement meetings as well as in-service training opportunities 
  • May provide clinical supervision to unlicensed behavioral health clinicians and master’s degree students as assigned
