About The Position

The Intake Support Coordinator assists individuals and families referred to TRE for case management services by guiding them through eligibility determination, enrollment, and assignment to long-term services and supports (LTSS). This position ensures compliance with Health Care Policy and Financing (HCPF) regulations and timelines, fosters strong customer service, and builds foundational knowledge of case management best practices to prepare for advanced roles within CMA. The role involves providing administrative and coordination support to the Service Coordination team, maintaining accurate documentation, and supporting seamless transitions into ongoing case management services.

Requirements

  • Bachelor’s degree in human behavioral sciences or equivalent combination of education and relevant LTSS experience.
  • Minimum 5 years of experience working in Medicaid, intake, or human services.
  • Familiarity with HCPF regulations, Colorado Medicaid waiver programs, and case management processes.
  • Strong interpersonal, organizational, and communication skills.
  • Demonstrated commitment to equity, inclusion, and cultural responsiveness.
  • A valid Colorado driver’s license (unless exempt due to military status), reliable personal transportation, and minimum insurance coverage of $100,000/$300,000/$100,000 are required.

Nice To Haves

  • Experience with case management or intake coordination in a nonprofit or human services setting.
  • Working knowledge of Medicaid Waiver programs and Health Care Policy and Financing (HCPF) rules.
  • Experience assisting individuals with disabilities and coordinating community services.

Responsibilities

  • Conduct intake interviews and assist individuals through HCPF eligibility screening and enrollment processes.
  • Coordinate documentation for level of care determinations, financial eligibility, and enrollment.
  • Generate and distribute application packets, schedule intake completion meetings, and facilitate functional eligibility assessments (ULTC 100.2).
  • Explain TRE services and Medicaid waiver programs to applicants to ensure program fit.
  • Maintain communication about all Medicaid waiver services, coordinate transfers to/from TRE waitlists, complete waitlist contacts, and manage termination of services notices (803) as per regulations.
  • Accurately document client interactions, eligibility determinations, and service activities in the Case Management System (CMS) and other required databases (BUS, CCMS, BILI).
  • Complete Per-Member Per-Month (PMPM) activity reports and adhere to DDD rules, regulations, and program quality standards.
  • Maintain up-to-date knowledge of Developmental Disability Determination processes, Long-Term Care Medicaid eligibility, and TRE’s billing methods.
  • Conduct detailed research and manage trackers related to financial eligibility and service utilization.
  • Correspond with external agencies to update applicant information and manage referrals.
  • Provide excellent customer service by responding promptly to emails and phone calls, following HIPAA and confidentiality guidelines.
  • Maintain positive and professional communication with clients, families, supervisors, and interdisciplinary teams.
  • Attend departmental and agency meetings, trainings, and professional development opportunities using various technologies (Outlook, Office 365, Zoom, etc.).
  • Provide coverage for other TRE positions as needed, including occasional after-hours availability.
  • Assist with special projects and support other Service Coordinators with administrative tasks.
  • Perform driving duties as related to the position, maintaining required licenses and insurance.
  • Ensure availability during customary TRE business hours with flexibility as needed.
