About The Position

The Intake Specialist is responsible for providing a prompt, warm, and respectful response to all incoming referrals. This role gathers the first pieces of information about a person who may join PACE and works closely with Enrollment Specialists, county partners, and Single-Entry-Point agencies to help move the enrollment process forward in a timely and compliant way. The Intake Specialist helps families understand how PACE works, offers referrals to community resources, supports community outreach and education efforts, and assists the Eligibility Specialist with Medicaid redeterminations and functional recertifications. This role promotes the PACE model in a kind, honest, and supportive manner that builds trust with older adults and caregivers.

Requirements

  • High School Diploma, required.
  • 1+ years of working in healthcare, required.

Nice To Haves

  • 2-3 years working in healthcare intake, eligibility, and/or enrollment (Highly preferred)
  • Experience working with Medicaid and or Medicaid Long Term Care (Preferred)
  • Experience working with the frail or elderly population (Preferred).
  • Experience with team-based care or IDT Models (Preferred)
  • Experience with Electronic Medical Records (EMR). (Preferred)
  • Bilingual in Spanish (Preferred)

Responsibilities

  • Participates in the PACE QAPI program by identifying improvement opportunities and supporting quality projects.
  • Identifies, responds to, and helps resolve issues in collaboration with the Manager.
  • Keeps the Manager updated on concerns, trends, or barriers.
  • Builds and maintains strong relationships with participants, caregivers, coworkers, and community partners.
  • Protects privacy and keeps all participant and organizational information confidential in accordance with HIPAA and PACE regulations.
  • Receives referrals (phone, walk-in, and online), provides simple and clear education about PACE, and screens for basic eligibility.
  • Enters all referrals into Zoho and PACElogic.
  • Assists with scheduling enrollment home visits.
  • Educates prospective participants and caregivers on required documents and steps in the enrollment process.
  • Schedules and completes PACE Center tours.
  • Maintains potential and current participant files.
  • Scans and uploads documents into participant files and PACElogic according to department standards.
  • Assists with disenrollment processes including death notification to county.
  • Completes monthly New Participant Orientation and All Agency Orientation.
  • Creates Participant Insurance cards and badges
  • Assists Eligibility Specialists with Medicaid Redeterminations and Functional Recertifications, including gathering documents, making reminder calls, supporting timely submission to prevent loss of coverage and follows up through re-approval.
  • Supports community outreach and education efforts, including attending community events, meeting with referral sources, sharing program information, and helping track outreach interactions.
  • Follows referrals through completion based on Policies and Procedures.
