About The Position

The Intake Clinician functions as part of a multidisciplinary team to serve the needs of potential and existing consumers, family, network members, and other stakeholders contacting or reporting to the agency for the purpose of receiving or navigating CC behavioral health services. The Intake Clinician delivers direct services to support the enrollment of new consumers into the CC system of care as well as deliver services to existing consumers as a part of the outpatient services department. All treatment activities are performed under the supervision of an independently licensed clinician and within the scope of the individual’s practice and training. This role carries a case load of adult consumers. Services are provided on-site and, in the community, as needed. This is a full-time position.

Requirements

  • Minimum Licensed Graduate Social Worker or Licensed Graduate Professional Counselor.

Responsibilities

  • Tracks and monitors communication from intake email, voicemail, walk-ins, and after hours call center to support the screening and scheduling of new admissions and the reengagement of existing consumers.
  • Pre-screens consumers by phone, video, or in person. Screenings consist of evaluation of need for care, eligibility, motivation for treatment, and insurance coverage.
  • Provides direct clinical services to consumers including triage, psycho-social assessments, diagnostic assessments, community support, DLA-20s, annual assessments, treatment planning, urgent or emergency service response.
  • Makes referrals as needed.
  • Completes enrollment into the electronic medical record; verifies insurance, reviews consent for treatment and other consent/acknowledgement documents, completes registration, completes initial clinical assessment, treatment plan, crisis plan, DLA-20, and orientation to the agency at-large.
  • Evaluates clinical progress and modifies method and intensity of clinical service as indicated by consumer’s progress and reaction.
  • Completes documentation and billing for all services in a timely manner and according to established standards.
  • Develop resources and relationships with external stakeholders and network members to improve referral opportunities.
  • Supports the referral process for consumers internally and externally for services as needed. Including but not limited to tracking referrals and following up accordingly.
  • Teams with physicians, nurse practitioners, clinical supervisors, community support specialists, administrative support staff, and pharmacy staff to coordinate care as applicable.
  • Care coordination - communicates with external agencies and other treatment providers, especially co-located service providers, as needed.
  • Performs other administrative duties as assigned for the overall system of care, including but not limited to enrollments and discharges, assignments and transitions in care.
