Intake Specialist
About The Position
Hospice Intake Specialist Farmingdale, NY Good Shepherd Hospice, a member of Catholic Health, promotes the physical, emotional and spiritual well-being both of the person completing life's journey and their family members, providing comprehensive and compassionate Palliative and end-of-life care of the highest quality. Good Shepherd helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most. The Hospice Intake Specialist responds to inquiries regarding our Hospice program and philosophy, verifies the appropriateness of the referral via physician certification, coordinates community and facility admissions, and maintains statistics on referrals, admissions, source of referrals and quality assurance indicators.
Requirements
- High School Diploma required.
- Strong computer skills, including Microsoft office and Electronic Medical Records.
- Strong organizational skills and attention to detail
- Excellent customer service and communication skills
- Ability to communicate with patients and their families in a compassionate and empathetic manner
- Prior experience in Healthcare preferred
