POSITION SUMMARY Under the supervision of the Director of Resource and Referral the Intake Specialist works to connect individuals and families with essential community resources both within the FREE network and in the larger community. The Intake Specialist utilizes the appropriate intake forms as well as the Accountable Health Communities Health-Related Social Needs (AHC HRSN) Screening Tool to collect member consent, documentation, demographic information, and contact details. Working with the Director of Resource and Referral the Intake Specialist will determine priorities and identify barriers to care and coordinate access to supportive services.

Requirements

  • High School Diploma or equivalent required, associate’s degree preferred
  • Must be at least 18 years of age or older
  • Valid Driver’s License and satisfy criteria to be an agency driver required.
  • No convictions for crimes that preclude a person from working with members of a vulnerable population
  • Proficient computer skills
  • Functional proficiency in spoken and written English
  • Demonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essential
  • Must be efficient, well organized, and have excellent organizational and interpersonal skills.
  • Ability to communicate effectively both verbally and in writing.

Nice To Haves

  • Experience working with Medicaid, OPWDD, and/or OMH population preferred
  • Knowledge of OPWDD, OMH, and Medicaid regulations preferred
  • Familiarity with community resources and linkages preferred

Responsibilities

  • Utilizes the Accountable Health Communities Health-Related Social Needs (AHC HRSN) Screening Tool to collect member consent, demographic information, and contact details.
  • Conducts screenings with Medicaid members utilizing a designated screening tool as per the respective Social Care Network.
  • Identifies and prioritizes Medicaid members for outreach and conducts required screenings.
  • Documents service provision, daily notes, and other information as required into the respective electronic health record.
  • Advocates with and for the people we support in a variety of situations.
  • Assists with connecting individuals and families with essential community resources, including but not limited to, housing, healthy food, transportation, and personal safety services.
  • Supports all operating divisions by coordinating admissions, internal transfers, and discharge processes to ensure smooth transitions.
  • Maintains accurate and up-to-date information in the EHR and database systems, ensuring correct program assignments for all individuals supported.
  • Supports the Director with coordination and attendance at all agency affiliated outreach events.
  • Reviews and evaluates all applications for admissions consideration.
  • Works collaboratively with all divisions throughout the agency to fill vacancies and problem solve internal movement.
  • Establishes and maintains appropriate correspondence with referring agents and document necessary information.
  • Performs screenings for new applicants as needed.
  • Tours families when requested.
  • Ensures that all required documentation (paper and electronic) is completed/processed appropriately, in a timely manner, and meets quality and compliance thresholds.
  • Performs other duties as assigned.
  • Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
  • Complies fully with FREE’s confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees.
  • Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk.
  • Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
  • Meets all job requirements and stays in compliance.
  • Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.
