Overview: Job Description Summary: Coordinates and assists in the provision of health resources and services to clients. Job Description: Essential Functions: Assists families in determining needed resources and services through a screening and triage process. Answers questions and concerns of families and provides service referrals and scheduling. Gathers referral and pre-admission information and completes all pre-admission data entry. Identifies emergent or complex cases and seeks clinical review prior to finalizing the referral. Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services. Utilize Electronic Medical Record and Scheduling software to assess the demand for services and the ability of current service capacities to meet the community’s needs. Maintains confidentiality of all clinical and administrative information consistent with agency policy.
