Shift: Monday - Friday 8:30 - 5:00 Job Location: 113 W. Drinker St. Dunmore, PA 18512 Full-time position: LPN to field all phone inquiries and take complete referrals for Home Health and Hospice services. Intake nurse is a key position for building relationships in the community with our referral sources. Excellent customer service and communication skills are required. Requirements

Current Pennsylvania LPN license
Home health background preferred
Strong interpersonal skills
Positive, proactive attitude

Responsibilities

Answers phones professionally
Takes all necessary information on referrals to insure all demographics, diagnoses, and medical orders are complete
Makes initial determination if patient can be accepted based on insurance type, patient location, and ability to staff
Collaborates with clinical team to insure all accepted referrals are processed for admission and scheduling
Process referrals and verify insurances
