About The Position

Concerto Renal Services is an industry leader in sub-acute hemodialysis within a skilled nursing facility and/or long-term care facility. We are a rapidly expanding business with a simple mission: reimagining dialysis care, one community at a time. Our model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model. The Intake Liaison serves as the central point of coordination for all incoming admissions, acting as the “air traffic controller” for referral flow. This role owns real-time visibility into incoming referrals, prioritizes urgent needs, and ensures timely communication and follow-through across teams. In addition to supporting intake operations, this role bridges communication between the offshore intake team, internal stakeholders, and SNF partners—ensuring escalations are surfaced, addressed, and resolved efficiently. Reporting to the Senior Director of Strategy, this position plays a critical role in improving responsiveness, maintaining service levels, and enhancing the partner experience

Requirements

  • High School Diploma or equivalent.
  • Experience in a fast-paced, high-volume coordination, admissions, or customer-facing environment strongly preferred
  • Must be proficient in typing and data entry.
  • Excellent oral and written communication skills.

Responsibilities

  • Monitor all inbound referral channels (phone, email, Teams, Salesforce) to maintain real-time awareness of referral volume and urgency.
  • Identify and prioritize urgent admissions, including same-day dialysis needs and escalated partner requests.
  • Ensure high-priority referrals are surfaced and acted on within established response time expectations (target: 1–2 hours).
  • Reprioritize workstreams as needed to ensure urgent cases are addressed promptly, and no referrals are missed.
  • Coordinate the collection and tracking of required clinical documentation for admissions approval.
  • Partner with social work and clinical teams to obtain necessary records in a timely manner.
  • Support documentation workflows for new unit openings and ongoing admissions as needed.
  • Support referral processing and data entry during absences or high-volume periods to help maintain SLA targets.
  • Partner with the intake team to address bottlenecks and improve overall intake efficiency.
  • Partner with the Intake Manager to support tracking and reporting of key intake metrics, including referral volume, response times, and workflow bottlenecks.
  • Help identify trends and areas for improvement to enhance intake performance and partner experience.
  • Serve as a bridge between various Concerto teams, including intake, operations, and billing to ensure timely communication.
  • Support billing-related follow-up, including missing SSNs, coverage changes, and patients without active insurance for facility invoicing.
  • Adapt fluidly to shifting departmental priorities, bringing a solutions-oriented mindset to emerging administrative needs across admissions and provider credentialing.
  • Provide backup support to the credentialing department by processing provider credentialing requests during absences or high-volume periods.
