Our model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model. The Intake Liaison serves as the central point of coordination for all incoming admissions, acting as the “air traffic controller” for referral flow. This role owns real-time visibility into incoming referrals, prioritizes urgent needs, and ensures timely communication and follow-through across teams. In addition to supporting intake operations, this role bridges communication between the offshore intake team, internal stakeholders, and SNF partners—ensuring escalations are surfaced, addressed, and resolved efficiently. Reporting to the Senior Director of Strategy, this position plays a critical role in improving responsiveness, maintaining service levels, and enhancing the partner experience
