Intake Coordinator
About The Position
LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home. The Intake Coordinator is responsible for facilitating participants/family enrollment to the LIFE Programs. This position will serve as primary contact with referral sources, participant/family from the time of initial inquiry until participant/family has been appropriately enrolled.
Requirements
- 1 to 2 years related experience and or training or equivalent combination of education and training.
- Working knowledge of computers, Microsoft Word and Excel required.
