The first friendly voice of care. Our Intake Coordinator is the welcoming front door to Health at Home. This role is often the very first interaction a patient, family member, or referral partner has with our organization, and that makes it incredibly important. You are the calm in the chaos, the organized one behind the scenes, and the friendly voice that helps people feel supported, heard, and confident in their next steps. You connect referrals to care, questions to answers, and families to the services they need, all while keeping things accurate, timely, and compliant. Requirements

  • High school diploma or equivalent required. Healthcare-related education is a plus.
  • Experience in healthcare intake, referrals, admissions, or insurance verification is preferred.
  • Familiarity with home health and or private duty workflows is strongly preferred.
  • Comfort with insurance verification processes and medical terminology.
  • Strong organization and time-management skills with the ability to juggle multiple referrals.
  • Excellent communication skills with a warm, professional phone presence.
  • Ability to work independently, use good judgment, and collaborate in a fast-paced environment.

Responsibilities

  • Receive and process incoming Home Health and Private Duty referrals from hospitals, physician offices, families, and community partners.
  • Answer private duty inquiry calls, gather initial information, and route leads to the right team to support timely care and conversion.
  • Review referral documentation for accuracy and completeness and follow up when information is missing.
  • Communicate referral status, next steps, and service availability clearly to patients, families, referral partners, and internal teams.
  • Verify insurance eligibility and benefits according to payer and agency guidelines.
  • Document coverage details, limitations, and authorization requirements accurately.
  • Explain benefit information in a clear, respectful, and easy-to-understand way.
  • Work closely with clinical, scheduling, operations, and sales teams to move referrals smoothly into active services.
  • Coordinate with leadership and clinicians on referral appropriateness, timing, and care needs.
  • Serve as a bridge between referral sources and internal teams to ensure responsiveness and service excellence.
  • Provide timely, compassionate, and professional communication to patients, families, and partners.
  • Maintain strong relationships with hospitals, physician offices, discharge planners, and community partners.
  • Represent Health at Home’s values, professionalism, and brand in every interaction.
  • Enter and maintain referral, intake, and benefit information in our electronic systems.
  • Keep organized referral records and communication logs.
  • Monitor referral pipelines and follow-ups to support timely service starts.
  • Protect patient privacy and comply with HIPAA and company policies.
  • Follow all federal, state, and agency regulations for home health and private duty services.
  • Adhere to all standards within the organization’s Compliance Program.
