About The Position

Position Summary: The Intake Coordinator I communicates and coordinates with patients, families, and physician offices. Process referrals and coordinate patient care. Assist to ensure smooth clinical operations. Maintain strong community relationships, and helps ensure adherence to regulations, policies, and procedures. The Intake Coordinator I will typically have experience in the Home Health/Hospice industry but need not be licensed in a skilled discipline. Responsibilities

• Respond to incoming phone calls to handle referrals, scheduling, and coordination of details to support field staff, patients, families, and referral sources.
• Build and maintain strong relationships with referral sources and other community contacts.
• Assist field staff with information, scheduling, and workflow.
• Responsible for Homecare Homebase workflow appropriate to the Intake roles.
• Work with the office team to provide other types of office support as needs dictate
