About The Position

Responsible for the daily intake of Hospice, , Palliative Medicine, and LDHH referrals; coordinates the flow of phone calls, faxes, and medical information related to these referrals. Assists callers in a courteous and professional manner while presenting VIA Health Partner's mission and services. Utilizes resources, such as the Interdisciplinary team, to meet the needs of the customers questions or needs. Works closely with fellow triage staff and administrative support to ensure all referrals are entered in the Clinical Software System accurately and timely. Schedules and prioritizes assessment visits and anticipating patient needs. Requirements

  • For non-clinical, a High School diploma or GED is required.
  • For clinical, a Diploma or associate degree required – Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse.
  • Current, active, and unrestricted license to that state the employee is assigned to work.
  • Current CPR required.
  • Minimum of two years of related experience.
  • Experience in direct care of patients with life limiting illnesses required.
  • Related experience may include hospice and palliative care, family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
  • Computer proficiency is required.
  • Excellent interpersonal and communication skills (both written and verbal).
  • Strong problem solving and critical thinking skills.
  • Well organized, self-directed, and able to manage time.
  • Flexible, innovative, ability to multi-task, resourceful and creative.
  • Ability to interact with patients and family members in varying psychosocial circumstances complicated by the presence of terminal illness.
  • Ability to be diplomatic yet assertive.
  • Excellent customer service skills and commitment to quality patient care.
  • Knowledge of nursing principles, practices, and policies, and sound clinical skills, if applicable.
  • Knowledge of medical terminology.
  • Understanding of hospice and/or home healthcare policies, and procedures.
  • Strong belief in hospice mission.
  • Excellent ability to work within a team.
  • Excellent ability to coordinate and foster multi-interdisciplinary care.
  • Strong computer skills with competence in electronic intake and phone triage.
  • Ability to deal with a variety of variables under limited standardization.
  • Able to interpret various instructions.
  • Ability to perform basic math skills, use of decimals to compute ratios and percents, and to draw and interpret graphs.
  • Ability to read a variety of books, magazines, instruction manuals.
  • Ability to prepare memos, reports, and essays using proper punctuation, spelling, and grammar.
  • Ability to communicate distinctly with appropriate pauses and emphasis, correct pronunciation (or sign equivalent), and variation in word order using present, perfect, and future tenses.
