Requirements

Associate's degree in counseling, social work or related human service field required.
Background check clearance required.
Demonstrated proficiency using electronic health record and client tracking systems.
Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
Valid driver's license, in good standing for the state of residency required.
Access to reliable transportation required.
Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.

Nice To Haves

Experience with intake assessment and scheduling client appointments preferred.

Responsibilities

Respond to all referrals from the CCS Case Work Team.
Refer clients that do not meet criteria for LSSI services to care coordination for evaluation and Image to additional resources.
Verify benefits and match insurance coverage to the appropriate service or provider.
Maintain knowledge of LSSI programs and alternative community resources.
Manage influx of new clients and program size.
Schedule intakes and referral assignments with appropriate counselor.
Utilize Electronic Health Record and client tracking systems.
Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizationsand/or individual in a dignified, respectful, honest, and fair manner.
