About The Position

The Intake Coordinator is primarily responsible for gathering demographics and historical information to assess the clinical needs and urgency of clients requesting behavioral health services and diagnosis. Your expertise will help tailor individualized treatment plans that address the unique challenges of each client, contributing significantly to their recovery and their overall well-being, complete authorizations in accordance with  established professional and ethical guidelines and policies and procedures.   Interrelationship: Will interact and communicate with Clinical staff and support staff as it relates to required services.

Requirements

  • Must have excellent verbal and written communication skills, active listening skills and exceptional customer service skills to evaluate clients, assess their care needs and guide through the admissions process.
  • Proficient with data entry and other technical computer skills requiring information to be entered into digital systems, accessing digital forms, and creating electronic patient files.
  • Must have excellent time management skills to schedule appointments for patients and manage multiple patients at once.
  • Commitment to ethical practice and adherence to all professional and legal confidentiality guidelines
  • Master’s degree in Psychology, Social Work, Counseling, or a related field.
  • Current, valid state licensure as a clinical psychologist, LCSW, LPC,LMFT or Associate licensure (LMSW, LAPC, AMFT).
  • Minimum of two years experience in clinical therapy, with demonstrated proficiency in clinical assessment and treatment.
  • Experience with electronic health records and clinical documentation software.

Responsibilities

  • Provide telephone and in-person triage, crisis evaluation, and crisis management for consumer to assist in prioritization, scheduling and assignment of new clients.
  • Conduct intake interviews, including administration and interpretation of client questionnaires, and medical/psychological history
  • Performs thorough biopsychosocial assessment and develop and implement client driven treatment plans using evidence-based therapeutic techniques suited to client needs and goals.
  • Conduct diagnostic evaluations to understand client needs and challenges.
  • Assign clients to clinicians or group therapy and prepare intake summary and write-ups using the electronic records system.
  • Coordinate services, including consultation with psychiatrist, referral to appropriate community resources, follow-up, and case disposition.
  • Maintain clinical records in accordance with agency policies and state/federal regulations.
  • Participate in scheduled meetings, including staff meetings and clinical meetings.
  • Participate in trainings to maintain and enhance job skills.
  • Perform, as assigned, other responsibilities associated with counseling and consultative services including, but not limited to participation in committees and Implementation of research activities and special projects.
