About The Position

We are seeking a compassionate, capable, and caring Full-Time Intake Coordinator to join our team at our corporate office in Long Beach. The Intake Coordinator will play a key role in the conversion of new referrals, guiding clients through the onboarding process, and building strong, ongoing relationships with a variety of referral sources to ensure a smooth and supportive intake experience. Available: Monday-Friday This is not a remote position

Requirements

• 18+ years old
• US work authorization
• High school diploma or equivalent
• Ability to pass a background check and reference check
• Valid Drivers License and reliable transportation

Nice To Haves

• Preferred bachelor’s degree
• Effective verbal and written communication
• Ability to handle a fast-paced environment
• Capable of handling a high volume of calls
• Fluency in both English and Spanish preferred

Responsibilities

• Conversion of New referrals
• Achieve referral conversion according to department standards
• Timely outreach based on company guidelines
• Maintain up-to-date client status to share with referral sources
• Partner with referral sources to obtain updated client information
• Effective Client Onboarding
• Educate clients on services for successful transition to care
• Document activities as directed
• Share information with staffing to have a successful caregiver match
• Relationship Building
• Reply to referral emails in a timely manner
• Update referral sources on status of client/family outreach
• Maintain referral source portals with up-to-date information
