In addition, this position develops preliminary determinations of an applicant’s eligibility for Center for Recovery and facilitates the admissions process for eligible individuals. The Intake Coordinator plays a critical role in coordinating admissions for incarcerated individuals, including those referred to through New York State Department of Corrections and Community Supervision (NYS DOCC) and Orange County Jail facilities, ensuring timely documentation, and compliance with agency and regulatory requirements. Under the supervision of the Director, the Intake Coordinator is responsible for contributing to the effective day-to-day operation of Admissions, ensuring that all screenings and admissions are conducted according to agency policy and procedures and provides as needed coverage/assistance to the staff. Additionally, the position carries a caseload of OTP participants. The Intake Coordinator will participate as a member of an interdisciplinary treatment team to facilitate the participant recovery and rehabilitation processes.

Requirements

  • Qualified Health Professional (CASAC Advanced, LMHC, LMSW, LCSW) and 1-year of substance use disorder experience or
  • Qualified Health Professional and 2 years’ experience in a related capacity in a social services setting
  • Must be computer-literate and understand Microsoft Word and Excel. Experience with electronic data entry is a plus.
  • Strong interviewing and assessment skills
  • Good working knowledge of community social service resources.
  • Ability to articulate in a clear and concise manner the agency’s eligibility criteria and its treatment continuum of care.

  • Bilingual (English/Spanish) is a plus.

Responsibilities

  • Conducts screening/intake interviews towards determining appropriateness of applicants seeking program admission.
  • Determines a level of care recommendation in accordance with agency eligibility criteria for supervisory review.
  • Enters all applicant screening/post-screening follow-up contacts into the agency’s electronic database.
  • Ensures that all required screening/intake or admission documentation are completed and processed as per agency policy.
  • Provides appropriate referrals to applicants needing crisis or other immediate community-based referrals.
  • Conducts initial phone screening assessments and maintains records of assessment referral status from initial contact as per OASAS and Joint Commission requirements.
  • Monitors and oversees the intake phone line and email inquiries, responding to voicemail and email messages from individuals requesting program information in a timely manner.
  • Conducts all intake and assessment scanning.
  • Enters any/all OASAS PAS forms into the Client Data System and ensures valid admission entry through the Central Registry.
  • Determines guest dosing eligibility and assists in providing services to guest dose requests from outside agencies, DOCCS, Orange County jails, along with monitoring guest dosing lists and reports.
  • Provides post-screening follow-up with assigned applicants on the eligibility and/or waitlist while monitoring the OTP waitlist and maintaining accurate, up-to-date records in the OASAS CDS system and OTP Excel waitlist.
  • Maintains strong working relationships with NYS DOCCS and Orange County Jail to provide appropriate SUD MAT services to incarcerated individuals as outlined in current DOCC’s contract.
  • Maintains an assigned caseload and ensures coordination with NYS DOCCS for the scheduling of counseling sessions conducted via Webex for all telemedicine appointments. Provides monthly analysis of admissions regarding census and referral sources.
  • Provides outreach and educational services to the local community, schools, groups and agencies as necessary.
  • Functions as a liaison/advocate to Participants’ families, legal authorities/representatives and/or other community-based agencies.
  • Ensures the maintenance of accurate, complete, timely and high-quality participant records that comply with external regulatory standards and agency policy and procedure.
  • Complies with 42 CFR confidentiality and HIPAA privacy and security regulations.
  • Attend and participate in weekly department, All Staff and other required meetings.
  • Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the CFH Policy and Procedure Manual and Employee Handbook.
  • Maintain confidentiality of all aspects of CFH including, but not limited to, participant confidentiality, financials, and employee relations.
  • Provide warm, friendly and welcoming environment upon initial contact with potential participants in addition to providing a warm hand-off to outside providers when necessary.
  • Performs other duties as assigned and needed.
