We are looking to hire an Intake Coordinator to join our Hayesville, NC home health team. This is Monday through Friday, on-site position at our Hayesville office. Requirements
- High school diploma or GED equivalent
- Prior clinical or non-clinical home health experience required
- Knowledge of Medicare, TennCare, state and federal regulations, and accreditation standards as they relate to clinical home health a plus
- Proficient in Microsoft Office Suite including Outlook, Work and Excel
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
- Strong initiative, follow through, process orientation, attention-to-detail
- Demonstrated confidentiality, professional and business acumen
Responsibilities
- The Intake Coordinator is responsible for accepting incoming calls for new referrals for clinical services provided to home care patients within an assigned geographical area.
- Receives clinical information on new patient referrals to the home health agency
- Collects all data pertinent to the request for admission to include demographic data, medical history, medication, physical, discharge summary, labs, physician orders and insurance information
- Verifies insurance/payer’s source via DDE or specific system
- Ensures referral acceptance and communication within 15 minutes of receiving referral
- Notifies clinical designee responsible for scheduling and authorizations
- Contact patient/clinical designee prior to admission to ensure service location
- Liaisons between field, medical, office and referral source staff
- Obtains authorization from insurance companies and ensures physician licensure compliance
- Conducts follow up on current active patient hospitalizations and communicate with clinical supervisor to obtain orders as needed
- Maintains non-admit log
