Position Summary We are seeking an experienced and compassionate Intake Coordinator to join our growing team. In this vital role, you will manage the patient intake process from start to finish, ensuring a seamless experience for patients, families, and referral sources. You’ll be the first point of contact and a key contributor to our mission of delivering life-changing service.

Requirements

  • Minimum 2 years of healthcare experience, preferably in home health or hospice intake/referrals.
  • Strong communication, negotiation, and public relations skills.
  • Demonstrated autonomy, flexibility, and teamwork.

Nice To Haves

  • Familiarity with EMR systems and insurance processes preferred.

Responsibilities

  • Oversee daily patient referral and intake operations.
  • Ensure compliance with state, federal, and Joint Commission regulations.
  • Implement and improve intake policies and procedures.
  • Build and maintain strong relationships with referral sources.
  • Coordinate patient education, plan of care initiation, and service provider communication.
  • Manage insurance verification and authorization processes.
  • Monitor referral trends and report key metrics.
  • Maintain knowledge of agency contracts and community resources.
