About The Position

We are seeking a detail-oriented and compassionate Intake Coordinator to support the intake and admission process for our home health and hospice agency. This role is critical in ensuring a smooth transition for new patients by coordinating referrals, verifying eligibility, and maintaining clear communication with referral sources, patients, and internal clinical teams. The ideal candidate thrives in a fast-paced healthcare environment, is highly organized, and is committed to delivering excellent service and patient care.

Requirements

  • Experience in an intake, admissions, or administrative role, preferably in home health or hospice
  • Knowledge of Medicare, Medicaid, and private insurance verification processes (preferred)
  • Strong organizational and time-management skills with the ability to manage multiple referrals simultaneously
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and EMR systems
  • Excellent verbal and written communication skills with a professional and compassionate approach
  • Ability to work independently, prioritize tasks, and adapt in a fast-paced environment
  • Spanish (Required)

Nice To Haves

  • Knowledge of Medicare, Medicaid, and private insurance verification processes (preferred)

Responsibilities

  • Manage incoming referrals and coordinate the patient intake and admission process
  • Verify insurance eligibility and benefits (Medicare, Medicaid, and private insurance)
  • Serve as a primary point of contact for referral sources, patients, and families
  • Ensure all intake documentation is complete, accurate, and compliant with regulations
  • Coordinate with clinical teams to facilitate timely start-of-care visits
  • Accurately enter and maintain patient information in the EMR system
