About The Position

Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, Care Management Specialist and Community Health Worker, etc.) The Intake Coordinator will serve as the first point of contact for all referrals to PMS Care Management. The Intake Coordinator will oversee the intake process from initial contact through the Health Risk Assessment and referral to a Care Management Specialist as needed.

Requirements

  • High school diploma or GED and minimum two years’ healthcare experience.
  • Self-starter with strong verbal and written communication skills.
  • Comfort with change and quality improvement processes.

Nice To Haves

  • Experience with Electronic Health Records strongly preferred.
  • Experience with Motivational Interviewing strongly preferred.
  • Knowledge of New Mexico and local community resources strongly preferred.
  • Bilingual (English / Spanish) or (English / Navajo) preferred.
