Requirements

  • H.S. Diploma or G.E.D. with up to three (3) years relevant experience or an equivalent combination of education and relevant experience.
  • Valid N.Y.S. Driver’s License
  • N.Y.S. Central Registry clearance.
  • Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.
  • Mobility sufficient to work in an office environment.
  • Manual dexterity sufficient to operate office equipment including machines.

Nice To Haves

  • Experience working with homeless population preferred.

Responsibilities

  • The Program Associate will facilitate the transport of individuals and families to shelter.
  • Conduct initial diversion/intake screening prior to contacting 211 to verify homelessness eligibility of individuals and families.
  • Support the quarantine placement of COVID individuals and families in DSS-contracted hotel.
  • Respond to all incoming calls from clients and/or providers.
  • Complete a diversion screening with walk-in clients prior to calling 211 to confirm homelessness eligibility.
  • Pick up clients and transport them to the appropriate shelter as determined by 211/DSS.
  • Support quarantine of COVID clients through proper documentation prior to their hotel placement, transportation to quarantine, and providing food, hygiene items, hotel rules.
  • Work with designated hotel agencies/shelters to provide a warm client hand off.
  • Maintain and update transportation logs and documentation via shift notes.
  • Communicate with other shift Program Associates/Caseworkers/admin as needed.
  • Ensure there is adequate staffing prior to leaving a shift.
  • Observes all safety rules and regulations including use of personal protective equipment.
