About The Position

The Inpatient Social Worker provides direct casework and counseling services to patients and their families within the Bluegrass Care Navigators’ Hospice inpatient unit and hospital settings. This role includes conducting comprehensive psychosocial assessments, identifying patient and family needs, establishing appropriate treatment goals, and implementing interventions to help alleviate psychosocial challenges.As an integral member of the interdisciplinary team, the Inpatient Social Worker collaborates in the planning and coordination of patient care and contributes to overall care delivery, except where limited by licensure. The Inpatient Social Worker works under the direction of the Counseling Manager and Executive Director, to ensure coordinated, patient-centered care that supports the physical, emotional, and social well-being of patients and their families.

Requirements

  • Graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council on Social Work Education (CSWE).
  • Must hold current licensure in Kentucky as a Licensed Social Worker (LSW), Certified Social Worker (CSW), or Licensed Clinical Social Worker (LCSW).
  • Minimum of two (2) years of social service experience, preferably in a healthcare setting.
  • Knowledge of the process of death, dying, and bereavement.
  • Understanding of the psychosocial and legal considerations affecting terminally ill patients and their families.
  • Knowledge of interpersonal dynamics within interdisciplinary healthcare teams.
  • Ability to exercise sound judgment and discretion when handling confidential information.
  • Demonstrates flexibility and strong written and oral communication skills, including the ability to give and receive constructive feedback.
  • Maintains a time-conscious attitude, consistently completing assignments efficiently, in an organized, self-directed, and professional manner.

  • Experience working within an interdisciplinary team is preferred.
