About The Position

Hospice Care Center is seeking a Part Time Hospice RN Case Manager (RNCM) to join their dedicated nursing team, providing exceptional end-of-life care throughout the community. This role involves delivering hands-on hospice nursing care, coordinating services, and guiding patients and families through their hospice journey. As a member of the interdisciplinary care team, the RNCM helps ensure patients receive compassionate, clinically excellent, and connected care. Requirements

  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.
