About The Position

Inpatient Clinical Nutrition Coordinator-Mount Sinai West The Clinical Nutrition Coordinator (CNC) provides experienced and quality nutrition care and education to patients and family members. This individual demonstrates advanced practice in a clinical focus area and recommends the appropriate nutrition therapy as it pertains to the patients’ nutritional needs. Additionally, the CNC coordinates departmental projects, initiatives, and education programs and acts as a resource to clinical staff, providing feedback for professional growth and development. The CNC adheres to department and hospital philosophies, standards, objectives, and policies and procedures

Requirements

  • Bachelor?s degree required, with concentration in Nutrition and Dietetics or related field from an accredited university.
  • Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions required.
  • Registered Dietitian (RD) from the Commission on Dietetic Registration (CDR)
  • Minimum 3 years of clinical experience

Nice To Haves

  • Master?s degree preferred but not required.
  • Advanced credential such as the Certified Diabetes Educator (CDE), Certified Nutrition Support Clinician (CNSC), or Pediatric, Renal, Oncology specialist preferred, but not required.

Responsibilities

  • Demonstrates advanced practice in clinical focus area.
  • Applies experience and knowledge of nutrition screening/assessment to evaluate specific patient populations.
  • Gathers the appropriate nutrition/medical history, and provides medical nutrition therapy by developing care plans that meet patients’ individualized nutrition needs.
  • Conducts nutrition-focused physical exams to determine risk and/or degree of malnutrition.
  • Documents pertinent findings in the medical record as per policy.
  • Maintains individual daily productivity of direct and non-direct patient care functions with timely submission to direct supervisor.
  • Meets department benchmarks for productivity.
  • Provides quality nutrition counseling to patients, family members, and caregivers as indicated.
  • Advocates on behalf of patients and families regarding nutrition care.
  • Evaluates nutrition care plans for effectiveness and recommends changes as needed.
  • Uses professional judgment to adapt and apply guidelines, protocols and professional standards of care to the new and changing needs of the patient.
  • Evaluates the need for enteral and parenteral nutrition support.
  • Provides recommendations for nutrition support per hospital and department policy.
  • Obtains patient food preferences and confirms allergies.
  • Communicates effectively with the diet office, as needed.
  • Performs meal rounds.
  • Works collaboratively and professionally with the Food Service Department to ensure patient needs and compliance to therapeutic diets are met.
  • Functions as a vital part of the healthcare team, actively participates in interdisciplinary rounds and assists with discharge planning.
  • Coordinates internship rotation(s), including schedules and student evaluations.
  • Leads instruction of competencies required to complete assigned rotation.
  • With the support of department leadership, coordinates training and development activities for clinical nutrition staff.
  • Orients new hires on department policies and procedures, and assists with professional growth and development.
  • Exercises independent initiative and judgment.
  • Coordinates projects within the department, including administrative (i.e. productivity reports, calendars and schedules, etc.), interdepartmental activities, lectures, and community-focused education programs.
  • Participates in external speaking events to promote service and education.
  • Assists department leadership with the design, implementation, monitoring and reporting of quality assurance and performance improvement plans.
  • Participates in performance improvement projects in the department.
  • Designs, develops, and regularly updates nutrition education materials to meet the specialized needs of specific patient populations.
  • Actively participates in department and hospital committees and meetings.
  • May participate in practice-specific nutrition research.
  • Accepts and incorporates feedback for clinical development
  • Encourages and motivates peers and colleagues.
  • Provides supportive feedback to clinical staff.
  • Ensures compliance with hospital/regulatory guidelines and policies.
  • Works rotating weekends and holidays.
  • Performs other duties as assigned.
