About The Position

Are you passionate about helping others and making a difference in your community? Conduent is seeking an Information and Referral Specialist to support individuals by connecting them with essential services and resources.

Requirements

• Bachelor’s degree in the human services field.
• At least 1 year in social or human services, ideally with elderly or disabled adults.
• Familiarity with Office of Aging and Adult Services (OAAS), Bureau of Health Services Financing (BHSF), Department of Health and Hospitals (DHH), Medicaid, Office for Citizens with Developmental Disabilities (OCDD), and home/community-based waiver services.
• Understanding of medical and developmental disability terminology; ability to maintain strict confidentiality, work independently, and remain composed under pressure; proficiency in Microsoft Office (MS Office).
• Onsite training in Baton Rouge (Mon–Fri, 8 AM–5 PM) for the first 6 weeks; hybrid work (Mon–Fri, 8 AM–5 PM) after training.

Nice To Haves

• Additional experience in social/human services with elderly or disabled adults.
• In-depth knowledge of OAAS, Medicaid programs, and related services.

Responsibilities

• Complete Level of Care Eligibility Tool (LOCET) assessments and other required evaluations.
• Manage inbound and outbound calls with professionalism and empathy.
• Provide accurate referrals to non-Office of Aging and Adult Services (OAAS) community services.
• Assess requestors’ needs and research available resources to meet those needs.
