About The Position

The Home based 0-5 Therapist works as a key member of an interdisciplinary team to provide a comprehensive continuum of home-based services for pregnant individuals and families with children birth to five. This role supports families across Wayne, Oakland, and Macomb counties, partnering closely with program staff and community providers to strengthen and preserve family wellbeing. The position focuses on identifying and responding to each family’s unique needs, enhancing parent–child relationships, promoting healthy pregnancy and early development, and coordinating services that foster resilience, stability, and thriving outcomes.

Requirements

  • Master’s degree in social work, psychology or other related human service field required.
  • Registration/licensure as a social worker, counselor, or psychologist in the State of Michigan required.
  • At least one year direct clinical experience in providing therapy services to young children and their families required (includes graduate internship experiences).
  • Current valid driver's license required.
  • Valid driver’s license and / or reliable transportation.
  • Must be COVID-19 vaccinated, or partially, with the ability to complete within 30-days of employment. Circumstances preventing vaccination, and corresponding accommodations will be reviewed and considered.

Nice To Haves

  • Infant Mental Health certification and/or endorsement preferred. Waiver to provided clinical services to birth to three population provided by employer to work towards endorsement.

Responsibilities

  • Provide parent-child relationship focused therapy and case management services within the home and community settings for pregnant women, fathers and young children birth to age six.
  • Conduct comprehensive assessments and treatment planning with measurable objectives.
  • Provide support and crisis intervention with families and children involved with the program.
  • Assist in coordination and referrals of support services for the family.
  • Provide parent education and guidance. Support overall child development.
  • Work cooperatively as a member of a team with program staff and community partners.
  • Participate in monthly marketing and outreach activities, develop referral relationships with other agencies and maintain those referral relationships.
  • Submit timely and accurate documentation of services, billing data and required paperwork.
