Requirements

  • Be a Social Work graduate with at least two years of Social Work or be willing to obtain a certification as a Social Services Designee.
  • Supervisory skills in a health care setting with at least two years’ experience.
  • Experience in geriatrics
  • Knowledge of State/Federal regulations regarding Resident Rights.
  • Fundamental knowledge of the difficulties and demands specific to the elderly.
  • Understanding of factors that have a potentially negative effect on psychosocial functioning.
  • Ability to perform the essential job functions with or without reasonable accommodations.
  • Basic knowledge of Excel and Microsoft Word

Nice To Haves

  • Some basic medical knowledge is preferred.

Responsibilities

  • Ensure the admission of Residents to the Apartments that are within the medical, financial, and psychological criteria of the facility, obtaining Advance Directives, durable power of attorney, Pre-admission OBRA (Omnibus Budget Reconciliation Act of 1987) screening, burial information, medical treatment and current physical, as well as provide guidance regarding Resident’s admission and discharge process as required.
  • Maintain an individual file for each Resident containing admission papers, financial data, burial Implement social services interventions to assist the Resident in meeting treatment goals.
  • Monitor the Resident's progress in improving physical and psychosocial functioning.
  • Know and follow the existing lines of communication and authority and serve as a role model in upholding the philosophy and objectives of the Home.
  • Establish and maintain good relationships with the Resident's family or legal representative.
  • Establish and maintain good relationships with people outside the facility to obtain necessary resources and services related to Residents' special financial, psychological, and social needs. arrangements, required state Medicaid forms, etc.
  • Maintain timely Residents’ assessments, documents and records as required by law/regulations and the Home’s policies and procedures, maintaining confidentiality of all Residents, department and organizational information.
  • Assist the administrative staff in evaluating, reviewing, and investigating family/Resident complaints and grievances.
  • Provide In-Service for the Independent Living Residents as needed.
  • Arrange logistic help as needed through internal and external sources. Ex. Arrange transportation of Residents to a hospital or other locations when necessary.
  • Be responsible for the supervision and performance management, direction, control, and planning of staff assigned to the Jeanne Jugan Pavilion and Independent Living Residents.
  • Assure that all Resident rights are maintained at all times.
  • Participate and serve on the Disaster Committee, and respond accordingly to Emergency System Gather rent checks for Development Office to deposit.
  • Work closely with a Little Sister who oversees the independent living apartments.
  • Maintain petty cash for Residents and reconcile as necessary ($1,500.00).
  • Develop an activity program monthly for the Independent Living Residents.
  • Perform other related work as directed by the immediate supervisor or Mother Superior of the facility Home.
