Have a working knowledge base of all GSIL programs, criteria for who is appropriate for a referral, and contacts for each program i.e., Peer Supports, Benefits etc. Meet with consumers in their homes and/or living spaces. Conduct in-person intakes: Evaluate home, vehicle, and community accessibility Collaborate with consumers to develop Independent Living Provide information about Independent Living services and goals. Encourage consumer self-advocacy efforts . Continuously identify and utilize local resources to assist consumers to remain independent within their communities. Provide proper referrals to individuals who wish to maintain or obtain employment . Empower consumers through: IL skills training: i.e., financial management, organization/household management, accessing public transit/ or conduct travel training, etc. Information to assist youth consumers to become independent adults. F acilitate Access Modification Program (AMP) process. Establish eligibility for appropriate funding sources such as Title VII, Part B or other AMP funding. Facilitate communication between consumers and vendors. Consult on project design/implementation. Assist with obtaining bids and verifying project completion. Submit accurate notes and reports within assigned times. Participate in Program driven outreach presentations and individualized outreach. Conduct other duties as assigned to increase choices for our consumers in their pursuit of independent living. Travel required Travel throughout the state is required. While performing the duties of this position, the individual is regularly required to sit, use a computer, a telephone, and other office equipment and to move throughout the building and attend meetings-in and out of the main office. Employees may occasionally need to exert up to 10 pounds of force to lift carry, push or otherwise move objects (e.g., files or presentation materials). Required education and experience. Minimum of an associate degree in social services, education, or a related field with at least 2-3 years of related experience required. Outgoing individual with ability to communicate well and organize work independently. Ability to provide information, direct services, and advocacy for consumers. Preferred education and experience A bachelor's degree in social services, education, or a related field with at least 2 years of related experience preferred. Personal experience with a disability preferred.
