About The Position

CareLinx by Sharecare is a nationwide caregiver registry that connects caregivers with clients based on their availability, preferences, and goals. Caregivers set their availability in the CareLinx app and are matched with clients for steady recurring shifts or per-diem flexibility. The role involves providing non-clinical, in-home care to health plan members and private clients. Once onboarded, caregivers work with a dedicated Care Partner team to find suitable matches based on location, client needs, and availability. The company emphasizes building consistent schedules, app engagement, and proactive communication with Care Partners.

Requirements

  • Minimum 6 months of caregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)
  • Access to a smartphone capable of running the CareLinx app (for scheduling, documentation, and timesheets)
  • Ability to communicate in English
  • Reliable transportation to and from client homes
  • Must pass a background check
  • Completion of program-specific training prior to accepting shifts (may apply depending on assignment)
  • Medical clearance such as TB test, physical exam, or drug screening (may apply depending on assignment)
  • Valid driver's license and MVR check if accepting driving shifts (may apply depending on assignment)
  • Additional documentation may be based on state law or client contract (may apply depending on assignment)

Responsibilities

  • Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
  • Complete and document all care tasks accurately in the CareLinx mobile app during each shift
  • Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
  • Submit timesheets on time through the CareLinx app
  • Communicate with your Care Partner regarding any case updates or client changes
  • Ensure patient privacy, dignity, and well-being in a safe and secure environment
  • Maintain client confidentiality at all times
  • Transportation and errand support (based on client needs)
  • Hospital discharge escort — safely accompanying members home after discharge (based on client needs)
  • Telehealth navigation — helping members connect with virtual doctor visits (based on client needs)
