About The Position

This role is crucial for delivering exceptional patient service across various clinical and community settings. It involves conducting critical health-related screenings directly with patients, documenting results, and communicating with patients about results. Priority screening areas include Cancer, HIV and Hepatitis, and health related social needs. These screenings will primarily take place in the Emergency Department setting and may also take place in other clinical and/or community settings. The position also includes navigating connection to needed services based on screening results and educating patients on prevention services and other resources. Regular communication with IMPACT staff and other relevant stakeholders as well as collaboration with the care team are essential to maintain program integrity, organized records and provide culturally humble interactions.

Requirements

  • Strong communication and interpersonal skills, with the ability to engage with diverse populations
  • Proficiency in use of Microsoft Word, Excel, and email required
  • Proficiency in data entry and familiarity with electronic health record (EPIC) systems preferred
  • Associate or bachelor’s degree required, and preferred that the degree be in public health, health education, nursing, social work, healthcare administration, or a related field.
  • Previous experience in healthcare, public health, case management or customer service is a plus

Nice To Haves

  • Experience working in a healthcare setting (including as a volunteer/intern) preferred
  • Phlebotomy experience preferred, not required
  • Bilingual skills (preferred but not required)

Responsibilities

  • Deliver exceptional service and resolve every transaction with the highest level of professionalism, ensuring a patient experience that reflects our unwavering commitment to efficient and effective care connections.
  • Demonstrate dedication to service excellence and consistent follow-through.
  • Identify patients eligible for various health-related screenings using electronic reports, manual review of medical chart, and direct discussion with patients
  • Conduct various health-related screenings onsite in various clinical and community settings for patients and community members.
  • Provide education on importance of early screening to patient population, primarily for various cancer types, HIV, Hepatitis C, and Hepatitis B.
  • Accurately document screening results in various data platforms, including REDCap, the electronic medical record (Epic), and Excel.
  • Provide direct services to patients, when relevant, such as training patients who to use tools such as the MyChart patient portal to enhance self-efficacy and health outcomes.
  • Deliver screening results to patients and provide targeted education on results, information on next steps, and counseling on potentially relevant resources.
  • Link patients with positive screenings to appropriate medical care, with support from IMPACT program specialists, other program staff, and clinical staff when relevant.
  • Schedule medical appointments for patients as needed.
  • Assist in development of patient education materials.
  • Ensure patients with unmet social needs are engaged with the Connected Care Services team to prevent barriers to follow up care.
  • Maintain regular communication with IMPACT staff and other relevant stakeholders.
  • Collaborate with the care team regarding screening results, interventions, and referral outcomes.
  • Ensure organized and secure record-keeping of screenings and timely data entry on activity tracking.
  • Contribute to program reporting.
  • Attend scheduled staff meetings and trainings and participate in discussions.
  • Apply cultural humility in interactions with clients and staff.
  • Operate with a high level of independence, as staff will often be working without other team members onsite across Enterprise locations
  • Demonstrate an understanding of how medical screenings are connected to other IMPACT initiatives
