About The Position

The Adult Skills Center (TASC) provides innovative and proactive services that empower individuals with diverse intellectual, developmental, and mental health needs to achieve their highest level of Independence and realize their greatest potential. The ILS/SLS Case Manager is under the supervision of the ILS/SLS Program Director and Program Manager. Must work effectively with other TASC Case Managers (if applicable) and be able to work successfully with minimal to no supervision. This position is responsible for the planning, development and implementation of daily program activities and objectives. Participates in program activities including direct support of clients and activities 75% of total work hours, assessment of clients, their goals, activity planning, and ensuring the health and safety of all clients. Must conduct verbal and written progress reports (ISPs). Significant community involvement necessary. In addition, is responsible for the supervision of TASC Direct Support Specialists (DSS). Primary Duties: 1. Direct Services 1. Provide Quality Assurance visits on daily basis as appropriate 2. Responsible to identifying coverage for clients as needed 3. Provide direct services to clients if no alternative coverage is found (hands-on instruction, training & support) 4. Respond to DSS phone calls in a timely manner 5. Provide training to DSS on Therap and eRSP (EVV) system during supervision visits 6. Mentoring of DSS including positive feedback and strategies on how to best support clients 7. Provide direct feedback/report on observations made during in home visits to supervisors 8. Work with individuals within their homes and community in order to increase their participation and independence in all their life’s activities 9. Complete Annual, Semi-Annual and Quarterly reports as needed 10. Ensure eRSP claiming and documentation is up to date 11. Prepare and clean-up of the areas of instruction 2. Program Coordination 1. Be available to monitor and assist selected instructors as requested by Supervisor 2. Be available to fill in for shifts when coverage is unavailable, at any time 3. Responsible for assisting in the training of instructors to ensure that the program and the staff are in compliance with State mandated requirements, (e.g., Title 17 and Title 22) 3. Participant Meetings (IPP’s, Annuals, Quarterlies, etc.) 1. Assist in the collection of all relevant data to present at meetings as directed by Supervisor 2. Assist in arranging for the participation of the participant and circle of support members at meetings as directed by Supervisor 3. Attend meetings and take notes to present to as requested by Supervisor, as needed or directed by Supervisor 4. Quarterly Circle of Support Meetings 1. Assist in the collection of all relevant data to present at meetings requested by Supervisor 2. Arrange for the participation of the participant and circle of support members at meetings as requested by Supervisor 3. Attend meetings and take notes to present, as requested by Supervisor 5. Health Support Responsibilities 1. Provide coverage for client if scheduled for medical appointments as requested by Supervisor 2. Assist Supervisor to ensure that participants attend medical appointments as requested by Supervisor 3. Be available to attend medical appointments as requested by Supervisor 4. Assist staff to ensure that medication is being ordered and complied with for participants as requested by Supervisor 6. Quarterly/Semi/Annual Reports Responsibilities 1. Assist in the collection of all data needed to write the reports with the lead staff person who is assigned 2. Assist in the preparation of and submittal of reports as requested by Supervisor 7. Professional Standards & Expectations 3. Embrace and model the principles and practices of person-centered thinking in all work activities 4. Understand and abide by TASC’s Mission and Values for the individuals served, including all policies and procedures 5. Maintain a positive attitude and creative approach that enhances the quality of life of individuals by having them participate in meaningful activities, always doing with and never for 6. Complete and submit all required paperwork per policies and procedures and funding source requirements 7. Provide immediate notification of any special incident or unexplained client absence to direct supervisor 8. Participate in all meetings and behavioral/skills training as required by TASC 9. Attend all mandatory ILS/SLS staff trainings and events and complete assigned trainings in Relias and other online sources 10. Support quality programming, health, and safety of all individuals in associated programs 11. Carry out the covenant of Do No Harm by ensuring the personal health and safety of all individuals in all activities 12. Professionally interact with Regional Center(s), DOR, IHSS, and other related organizations 13. Adhere to TASC and Regional Center quality assurance and compliance standards 14. Support fundraising activities for your program as required by your supervisor 15. Participate in all Coaching Plans, 1:1 Meetings and Performance Reviews by carrying out recommendations to improve competencies and skills 8. TASC Leadership 1. Represent TASC within the community 2. Participate on community boards and/or committees in order to enhance community knowledge of TASC as well as provide TASC with relationships with additional community resources 3. Speak to community groups when requested by Director 4. Participate in TASC strategic plan implementation 5. Participate in committees and contribute ideas 6. Foster a positive work environment through constructive feedback and problem solving 7. Encourage learning, personal growth, and positive attitude 9. Other Duties 1. Perform other duties as assigned 2. Perform other off-site duties as assigned 3. Participate in weekly meetings, trainings, etc. as assigned by the supervisor

Requirements

  • Evening and weekend work is required as needed.
  • Position requires a High School diploma/AA or BA Degrees preferred
  • Must have at least two (2) years of experience working with developmentally disabled individuals.
  • Must demonstrate the ability to work utilizing a team approach setting.
  • Must possess the ability to multi-task.
  • Must demonstrate the ability to work positively with or without supervision, while exhibiting excellent time management skills.
  • Must possess the ability to respond timely and appropriately to challenging situations
  • Maintain all certifications up to date
  • Must be computer literate in Microsoft Word and Outlook.
  • Must have good written and verbal communication skills, and good organizational skills.
  • All employees of The Adult Skills Center are required to have background checks and/or fingerprint clearance prior to hire.
  • Must have a clear DMV record.
  • Ability to work remotely (home/alternate location) upon request as necessary.
  • Ability to drive for work, maintain valid driver’s license and up-to-date car insurance.
  • Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
