The IDD Case Manager position provides assistance to eligible individuals in gaining access to medical, social, educational, and other needed services that will help the individual achieve a quality of life and community participation acceptable to the individual and/or LAR on the individual's behalf. This position's duties include the following: 1) Completes assessments to identify needs. 2) Develops an individual plan of care which documents the delivery of appropriate services and supports based on the individual's choices and preferred outcomes. 3) Completes CFC (Community First Choice) functional assessments, if applicable. 4) Works in accordance with MCO's (Managed Care Organizations). 5) Links, refers, and coordinates the services offered through Tri-County Services and community resources. 6) Monitors progress monthly to ensure quality and appropriateness of services. 7) Provides advocacy and crisis prevention/management as needed. 8) Documents services. 9) Assists with enrollments and intakes, as needed.

  • A high school diploma or a certificate recognized by a state as the equivalent of a high school diploma, and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
  • An Associate degree in a social, behavioral, human service, or health-related field including, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice; or
  • Bachelor's or advanced degree from an accredited college or university (ANY field).
  • For Bachelor's and Associates degree candidates, at least one year's paid or unpaid experience working with individuals with IDD is preferred.
  • Bilingual in Spanish & English a plus.
  • Must have a valid Driver's License and be insurable through Tri-County Behavioral Healthcare.
