About The Position

This position is located in the Wentworth office, Rockingham County which is a satellite office of the Burlington unit office. The Human Service Coordinator is responsible for providing direct case management services as indicated in the Individual Plan of Independent Living. The position is to work under the close supervision of the Rehabilitation Counselor Supervisor and Unit Manager. Job responsibilities include the following: Evaluating professional diagnostic data to determine eligibility and required services. Developing and implementing individualized plans of rehabilitation services for eligible consumers. Orchestrating and overseeing the provision of planned services. Interviewing consumers for the purpose of assessing need. Arranging for appropriate evaluations and other services indicated on the Individualized Plan of Independent Living. Engaging in various administrative duties including case service documentation, caseload management, staff and other administrative meetings. Participating in training and professional development activities.

Requirements

  • Candidates now meet the minimum qualifications of a position if they meet the minimum education and experience listed on the vacancy announcement.
  • Bachelor's degree in a discipline relative to the program served from an appropriately accredited institution; or Bachelor's degree from an appropriately accredited institution and one year of experience in an area related to program that provides experience in techniques of casework or counseling; or equivalent combination of education and experience.

  • Knowledge of federal and state laws governing the delivery of rehabilitation or independent living services
  • Demonstrated ability to interpret medical and psychological data in planning and developing rehabilitative services
