About The Position

Community of Hope is seeking a full-time Housing Stability Case Manager for our Rapid Rehousing Program. In this role, you’ll help families secure and maintain stable housing, increase their self-sufficiency through income, benefits, and education, and strengthen overall family stability. Requirements

  • Bachelor’s degree in social work or related field required or 2 years’ experience in a social services and/or human service field.
  • Minimum one year work experience in social service or related field required with degree. Minimum two years work experience in social service field if no degree.
  • Strong interpersonal, conflict-resolution, and organizational skills.
  • Ability to work independently and as a team.
  • A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.

  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly.

Responsibilities

  • Works with families using a two-generation (whole family) approach to develop individual goal plans that are measurable and detailed with action steps that will help each family achieve their goals. Updates goal plans according to program contract guidelines and as needed, signed by client and case manager.
  • Meets with families on caseload to support implementation of their goal plans and follow-up in accordance with program contract guidelines and family needs.
  • Networks and coordinates with community resources, makes referrals as appropriate, and follows up to confirm and track families’ participation in housing, employment, medical care, substance abuse treatment, mental health treatment, training, etc.
  • Assists families with housing search process in collaboration with Housing Specialist. As needed, accompanies families to view units, advocates for families with landlords, and assists families with applications and other paperwork. If COH is providing financial assistance for application fees, accompanies families to render payment.
  • Conducts scheduled and unscheduled home visits and provides tenant education in accordance with program contract guidelines, and as needed.
  • Maintains regular communication in accordance with program contract guidelines with all families on caseload and documents progressive engagement efforts with difficult to engage families.
  • Works consistently within a housing-first framework (i.e., providing clients with immediate access to available independent permanent housing and supportive services without prerequisites for sobriety or participation in psychiatric treatment), ensuring that services offered are strengths-based and client-centered.
