Provide housing retention services to individuals who are Medicaid enrollees who are street or shelter homeless and are enrolled in or eligible for Health Home care coordination. Assist the target population to establish and maintain housing stability and develop the skills needed to remain in stable housing and live independently. Foster an environment that assists clients to engage and maintain medical care. Services include but are not limited to facilitation of financial assistance, skill building, and coordination of services.
