Requirements

  • BA/BS degree preferred, High School diploma/GED required.
  • Minimum two years experience working with homeless, mentally ill, or MICA population preferred.
  • Knowledge of the NY/NY housing system.
  • Basic computer literacy required.
  • CPR training certification or willingness to take training class in CPR.

Responsibilities

  • Assist housing team in providing follow up services for one year to former clients placed into housing to ensure successful transition.
  • Act as liaison between the program and NY/NY Housing providers.
  • Facilitate psycho-educational groups for current and former clients.
  • Assist in placing clients in housing.
  • Provide comprehensive case management and services to a caseload of current clients.
  • Provide case management services to caseload of aftercare clients
  • Provide emergency first aid/CPR assistance when needed.
  • Perform additional related duties as assigned.
