About The Position

The Housing Provider Quality Liaison (HPQL) supports high-quality housing options for InnovAge participants by collaborating with Center IDT and corporate partners. This role ensures issues identified through audits and participant feedback are addressed appropriately, supports corrective action planning and implementation, and contributes to provider education initiatives. The HPQL works closely with Center staff but does not replace the responsibilities of assigned IDT members. Essential Functions and Work Responsibilities Functional Category: Housing Provider Quality Oversight 100% Supporting the Development, Implementation and Monitoring of Corrective Action Plans: Support development and execution of corrective action plans. Assign issues to appropriate Center staff and ensuring comprehensive resolution. Scheduling and Coordinating Partnership Meetings between InnovAge and Contracted Facilities: The HPQL, in collaboration with Center leadership and corporate partners, will facilitate Partnership Meetings with housing providers (assisted living, board and care, and nursing facilities) to include scheduling the meetings, creating the agenda, taking minutes, noting follow-up activities, and distributing the minutes to the meeting attendees. Additionally, the HPQL will be accountable for ensuring the follow-ups are executed upon by InnovAge and housing providers. Reviewing and Analyzing Provider Liaison Audit Reports: The HPQL will review Provider Liaison reports that include participant feedback to assess for required follow-ups. Analyzing audit reports to identify Service Delivery Requests, Grievances, allegations of fraud, waste, abuse or exploitation, and quality of care concerns. Document audit findings in Salesforce or other systems. This includes identifying grievances, entering them into Sales Force, and assigning IDT accountability for follow-ups. Supporting provider education initiatives. Performing other task,s as directed by the Center Director.

Requirements

  • Bachelor’s degree from an accredited college or university in an appropriate clinical discipline or public health. An RN/LPN or clinical degree/license preferred, or clinical equivalent.
  • Require personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law.
  • Applicable licensure for nurse applicants
  • Current CPR certification or the ability to obtain CPR certification is required.
  • A minimum of three years’ total related experience.
  • Two years’ experience in community-based social services organization or health care setting required.
  • Experience with health plan or healthcare provider quality programs
  • A minimum of one year experience working with older adults and people with disabilities

Nice To Haves

  • Three years or more of experience working in human, healthcare, or community-based services for older adults and people with disabilities.
  • Experience with analyzing and managing grievances.
