Manage caseload of households, providing ongoing support services including case management, goal planning, resource coordination, and advocacy to promote housing stability and self-sufficiency. Provide intake, screening, assessment, case planning, counseling, consultation, training, crisis intervention, and case management services. Secure housing, manage landlord relations, and facilitate intra-program and interagency communications for all units in supportive housing programs Screen, obtain, and monitor housing leases; maintain landlord/apartment database. Conduct home visits and unit inspections according to program requirements. Establish and monitor individualized case plans for clients. Maintain accurate case records, contractual paperwork, status reports, and fiscal records. Network with the community-at-large, advocate for clients, and make professional contacts. Communicate regularly with landlords, clients, case managers, and relevant stakeholders. Maintains awareness of the program budget, and functions within the budget guidelines for the program, ensuring spending does not exceed the defined guidelines. Other duties as assigned within the guidelines of this position. Requirements

  • Bachelor's degree in social work or related field required; Master's degree preferred.
  • Minimum of 1year of experience in an administrative or program position in a social services setting.
  • Ability to work effectively with diverse racial, cultural, family systems, and social backgrounds.
  • Ability to work with homeless and low-income populations.
  • Ability to handle complex cases and respond quickly in times of crisis.
  • Advanced proficiency in Microsoft Office applications (Excel, Word, Outlook, and related tools), including the ability to use technology to manage data, track deadlines, and support complex housing operations.
  • Strong organizational and workflow management skills, with the ability to independently manage multiple housing units, competing priorities, timelines, and detailed documentation requirements.
  • Experience using digital form-building, data collection, or workflow automation tools (e.g., JotForm or similar platforms) to streamline processes, standardize submissions, and improve operational efficiency.
  • Ability to analyze existing processes and develop or improve systems that enhance accuracy, accountability, and communication across teams.
  • Strong verbal and written communication skills.
  • Ability to work independently and maintain confidentiality.
  • Passage of background clearance, valid driver's license, reliable transportation, and proof of liability insurance.
