Apply now so we can make a lasting impact together! General Responsiblities Operate as a member of the Community Services Team. Manage caseload of households, providing ongoing support services including case management, goal planning, resource coordination, and advocacy to promote housing stability and self-sufficiency. Provide intake, screening, assessment, case planning, counseling, consultation, training, crisis intervention, and case management services. Secure housing, manage landlord relations, and facilitate intra-program and interagency communications for all units in supportive housing programs Screen, obtain, and monitor housing leases; maintain landlord/apartment database. Conduct home visits and unit inspections according to program requirements. Establish and monitor individualized case plans for clients. Maintain accurate case records, contractual paperwork, status reports, and fiscal records. Network with the community-at-large, advocate for clients, and make professional contacts. Communicate regularly with landlords, clients, case managers, and relevant stakeholders. Maintains awareness of the program budget, and functions within the budget guidelines for the program, ensuring spending does not exceed the defined guidelines. Other duties as assigned within the guidelines of this position.
