About The Position

Assists and prepares program participants in seeking housing and preparing for home ownership. This includes completion of an assessment with families in partnering to developing sustainability goals. Care coordination services including screening, intake, coaching, skill-building, developing and facilitating workshops, and referral to community agencies for participants. DUTIES SUMMARY Assists and prepares program participants in seeking housing and preparing for home ownership. This includes completion of an assessment with families in partnering to developing sustainability goals. Care coordination services including screening, intake, coaching, skill-building, developing and facilitating workshops, and referral to community agencies for participants. Engages participant in the completion of a tenant screening and housing assessment for the purpose of identifying housing needs, strengths, and barriers. Assists participant in developing a housing support plan while also collaborating with relevant contact area team members in reaching established goals. Assists with identifying, coordinating, securing, or funding one-time services and modifications necessary to enable a person to establish a basic household. Develops new housing agency contacts and maintains working relationships with current partners. The Housing Navigator utilizes hybrid schedule and works remotely and in the field to seek partnerships with housing agencies and/or supporting all participants. On an ongoing basis, the Housing Navigator must update the participant’s housing status and track on a web-based documentation and record keeping system. Coaches, assists, and trains participant individually and group settings on seeking, establishing, and maintaining housing and preparing for home ownership. Frequently develops, coordinates, and conducts related internal and external housing related workshops. Maintains and submits weekly and monthly participant/data rosters/reports which are submitted to supervisor and other related staff and departments. Collaborates with agency staff and community partners to optimize housing opportunities and desired results. Frequently collaborates with content team to ensure that all goals are being reached and all referrals and family goal follow ups are addressed in a timely manner. Housing Navigator carries a caseload of 20+ (varies based on referrals) Advocates on behalf of the participant with other agencies and government programs to aid in receiving needed services. Provides tenancy and sustaining services, with a goal of maintaining safe and stable tenancy once housing is secured. Sets up and maintains related files and documentation ensuring compliance with policies. Conducts data entry and record keeping in an electronic data base system Develops and distributes surveys in acquiring data related to participant needs and program outcomes. Attends internal and external collaborative meetings and professional development trainings. ADDITIONAL DUTIES Other duties and special projects as assigned. SUPERVISORY RESPONSIBILITIES This job has no supervisory responsibilities.

Requirements

  • High school diploma/GED required
  • Bachelor’s degree in social work, public health, psychology, or related field; AA in social work, psychology, or related field; and/or experience as a Housing Navigator and/or Whole Person Care or Health Homes Case Management experience required
  • Possesses a valid driver's license and state-required auto insurance.
  • Required to travel extensively (> 50% of the time).
  • Ability to work with minimal supervision.
  • Ability to handle confidential information.
  • Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
  • Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
  • Sensitivity to service population’s cultural and socioeconomic characteristics.
  • Excellent verbal and written communication skills.
  • Punctual and dependable attendance.
  • Must have intermediate knowledge of computer programs in a Windows environment.

Nice To Haves

  • Experience working with housing needs populations and/or navigating housing access systems and tenant support is preferred
  • Experience in the provision of case management services for persons living with mental illness, substance use disorders, homelessness, and/or legal system‐involvement preferred
  • Working knowledge of current housing practices for people experiencing homelessness
  • Spanish/ English bilingual preferred.
