Job Summary: The Housing Navigator provides onsite support to individuals and families experiencing homelessness or housing instability. Under supervision, this role assists with triage, intake, documentation, benefits applications, Coordinated Entry (CE) workflows, and accurate data entry into HMIS and Apricot. The Housing Navigator follows established program procedures, upholds safety and confidentiality, and delivers services in alignment with A New Leaf’s mission and funder requirements.

Requirements

  • High School Diploma/GED required.
  • Minimum 1–2 years of direct case management experience in a related field.
  • Must Be At Least 21 Years of Age
  • Class One Fingerprint Clearance Card
  • DCS Central Registry Background Check
  • Drug Screening
  • CPR/First Aid Certification
  • Therapeutic Options Training
  • Valid Unrestricted Driver’s License
  • Current Auto Liability Insurance
  • 39 Month Motor Vehicle Record

  • Must be able to maintain confidentiality and professionalism in all client and team interactions.
  • Ability to manage multiple tasks and cases

Responsibilities

  • Maintain a defined caseload under supervision, following established workflows and service plans.
  • Complete basic housing navigation plans, track participant action steps, and escalate barriers to the Program Manager or senior staff as needed.
  • Provide triage and intake services, identifying immediate participant needs and making warm handoffs to appropriate onsite providers or program staff.
  • Assist participants with obtaining required documentation such as identification, income verification, and other eligibility documents needed for housing or benefits.
  • Support participants with applications for entitlement benefits such as AHCCCS and SNAP using trauma-informed practices.
  • Connect participants to Coordinated Entry workflows, scheduling and updating assessments, and making referrals according to Continuum of Care (CoC) guidance and program policies.
  • Collaborate with community partners and housing providers as directed to support participant needs and case progress.
  • Maintain accurate, timely case notes and data entry in HMIS and Apricot, ensuring compliance with funder and program reporting requirements.
  • Maintain facility cleanliness and ensure equipment is safe and functional for staff and participants.
