The Social Worker provides engagement and supports program participants using a person-centered model that supports individuals in working through crisis, developing new coping strategies, assessing risk, maintaining their housing and developing and reaching goals that could include but are not limited to furthering their recovery, accessing community-based treatment and support services, connecting to community, family and social support networks, and exploring educational or vocational opportunities. The Social Worker supports other team members and housing staff by acting as a bridge between participants and resources, ultimately contributing to more holistic support. The Social Worker also stands as an advocate and liaison for program participants by representing their needs and rights, navigating complex systems, ensuring access to essential services, and promoting well-being and autonomy.
