About The Position

The Housing Liaison (HL) role is a client-facing position that serves as the bridge between participants, programs and community partners for individuals facing complex socioeconomic barriers. This role closes the loop between participants, providers and services both within and outside the agency by serving as a point of contact across multiple stakeholders. The HL receives referrals from Washington County Community Corrections divisions, including Parole and Probation and Community Corrections, as well as other embedded partner programs, including but not limited to Washington County Behavioral Health and New Narrative Spring Creek Clinic. This position is largely community based and works closely with Community Action Organizations (CAO), Washington County Department of Housing Services (DHS), the Washington County shelter network, Coordinated Access partners, Continuum of Care (CoC) housing providers, clinical teams, and other community stakeholders. Through both homeless outreach and in reach activities, the role supports participants with housing eligibility. It assists participants in connecting to supportive service resources as they transition from Parole and Probation, Community Corrections, and other embedded partner programs, including but not limited to Washington County Behavioral Health and the New Narrative Spring Creek Clinic, into housing. The Housing Liaison acts as a resource for individuals and households with low incomes (0–50% AMI), who have one or more disabling conditions and are experiencing or at imminent risk of long term or frequent episodes of literal homelessness in Washington County. The position ensures service delivery aligns with a trauma informed care approach, manages data to support program deliverables and goals required by funding and grant compliance.

Requirements

  • Maintain current Oregon driver’s license and required insurance
  • Participant Engagement & Professional Communication: Must be able to work independently, collaborate effectively with multidisciplinary teams and builds respectful, trauma informed relationships with participants, staff, landlords, and partners. Communicates clearly, maintains service boundaries and responds in a timely and professional manner.
  • Work Ownership & Adaptability: Demonstrates initiative, follows through on responsibilities, learns and applies new processes and adjusts to changing participant and program needs.
  • Critical Thinking & Problem Solving: Assesses situations, interprets information, identifies patterns and uses sound judgment to resolve housing and service barriers.
  • Documentation, Compliance & Confidentiality: Maintains accurate records, follows program requirements and procedures, uses data appropriately and protects sensitive information.
  • Professionalism & Crisis Stability: Remains calm and objective in crisis situations, demonstrates empathy and exercises consistent professional judgment.

Nice To Haves

  • Qualified Mental Health Associate (QMHA) and/or Peer Wellness Specialist (PWS)
  • Experience working in HMIS or Credible a plus.
  • Experience working with Harm Reduction programs a plus.
  • Experience with Housing First programing a plus
  • Familiarity with the population (SPMI, Justice involved, SUD, unhoused)
  • Bilingual proficiency is a plus.

Responsibilities

  • Maintain professional boundaries and strict confidentiality.
  • Practice assertive engagement to motivate participation and support follow through.
  • Coordinate referrals and complete client intakes, exits, and program placements while managing waitlists.
  • Serve as a liaison between participants and community partners by maintaining and expanding knowledge of community resources impacting people experiencing housing instability.
  • Conduct thorough assessments to identify each individual’s history, risk factors, and strengths to support successful outcomes.
  • Identify barriers to housing and services, such as transportation, documentation, and basic needs, and support short term problem solving through strength based, solution focused individualized action plans that promote income growth, housing stability and self-sufficiency.
  • Assist participants in accessing appropriate temporary housing options, including non-congregate shelter, congregate shelter, transitional housing and other interim housing resources.
  • Participate in internal and community case conferencing with embedded partner program staff to coordinate housing support, address participant needs, and conduct pre-engagement or engagement meetings to ensure coordinated transitions and warm handoffs.
  • The Housing Liaison role is largely community based, carries an active caseload (generally 15 to 30 individuals or households depending on program requirements) and provides short-term housing support services for each referral, adding new participants as openings become available.
  • Maintain accurate and up to date records of service encounters and enter data in a timely manner in HMIS, Credible, and/or Smartsheets using strong computer skills.
  • Gather housing documentation in an accurate and timely manner, including applications, disability verification, financial records, identification, and other documentation needed to support housing stability.
  • Respond to calls, emails, update requests, and other inquiries in a professional and timely manner.
  • Follow best practice standards of care and all New Narrative policies and SOPs, including release of information, informed consent, confidentiality, and file documentation.
  • Complete required training and certification within established timeframes.
  • Serve on agency or community service committees representing the department or agency as assigned.
  • Support the mission, vision, and values of New Narrative in all essential job functions and responsibilities.
