About The Position

Gardner Health Services is currently recruiting to fill two (2) Housing Lead Care Manager positions. These are full-time positions in based out of the Downtown Resource Center located at 777 E. Santa Clare St Suite 2004, San Jose, CA 95112. As a member of the Downtown Resource Center, Enhanced Care Management and Community Support program, the Housing Lead Care Manager (LCM) is working directly to support Non-Gardner and Gardner Health Services (GHS) high risk patients who are experiencing homelessness in addition of chronic disease conditions, serious mental illness, substance, and ongoing medical and social barriers. The Housing LCM will be part of an interdisciplinary team comprised of: primary care providers, RN Enhanced Care Manager, Care Coordinators/Lead Care Managers, Social Workers, and Health Educator/Coaches. In collaboration with the care team, the Housing LCM provides a variety of support to members enrolled in Community Support (CS)/In Lieu of Services (ILOS) for Housing Transition Navigation Services and Housing Tenancy and Sustaining Services.     This recruitment will remain open until filled. Requirements

  • Bachelor’s degree in Social Work or Psychology, required.
  • Ability to work with a diverse ethnic and multi-cultural population.
  • Knowledge of CPS and APS mandatory reporting laws.
  • Knowledge of medical terminology, Medicare, Medi-Cal, and county and community agency regulations.
  • Advanced communication skills in advocacy, negotiation and conflict resolution.
  • Computer skills including proficiency in medical practice management systems, internet use, and Microsoft office applications.
  • Knowledge and understanding of human behavior, development of personality, and adaptation to illness.
  • Knowledge effects of hospitalization on patients and families.
  • Excellent written and verbal communication skills are a must.
  • Ability to set and meet deadlines.
  • Creative and flexible team player.
  • Must be comfortable in a fast-paced environment.
  • Demonstrated working knowledge of OSHA and HIPAA guidelines

Nice To Haves

  • Two (2) years’ of experience in the provision of social services and/or discharge planning in an acute care setting, preferred.
  • Bilingual English/Spanish, preferred.
