About The Position
The STAY Program is seeking a compassionate, motivated, and enthusiastic Housing Coordinator to provide coordination and support services to individuals transitioning from chronic homelessness into permanent supportive housing. This role focuses on promoting residential stability for individuals with disabilities through strong collaboration with staff, landlords, and community partners.
Requirements
- Bachelor’s degree in Human Services or related field required
- Knowledge of chronic homelessness and related subpopulations
- Experience working with individuals with disabilities preferred
- Familiarity with mental health services and community resources
- Knowledge of housing/leasing processes preferred
- Ability to transport clients and conduct home visits
- Ability to coordinate community-based activities and outings
- Demonstrated ability to build strong community partnerships and maintain professional relationships
Responsibilities
- Serve as liaison between STAY staff, residents, and landlords
- Respond to all tenant maintenance requests within 24 hours
- Maintain proactive communication with property managers
- Identify and address landlord concerns before they escalate
- Conduct quarterly property inspections
- Monitor apartment conditions to ensure safety and welfare
- Maintain tracking systems for maintenance requests and follow-ups
- Complete timely and accurate case notes and program documentation
- Perform regular home visits and provide transportation as needed
- Attend regular training sessions and professional development opportunities
- Provide client-centered, strengths-based support services
- Coordinate housing stability efforts to support independent living
- Educate and advocate for consumers in accessing mainstream and community resources
- Demonstrate strong community engagement by building and maintaining relationships with local service providers and agencies
- Assist clients in meeting basic needs and strengthening natural supports
- Plan and coordinate outings and activities that promote socialization and community integration
